A BUSY PRACTICE

Regrettably, there are approximately 130 appointments lost each month, often due to patient's failure to attend.  This equates to more than 10 full surgeries each month.

If you know you are unable to attend, please phone to cancel, as this will free up the appointment for another person to make use of.

New Patient Registrationregister_now

If you live within our practice area you are welcome to register with us and our Reception Staff will be happy to guide you through the procedure. You will need to complete a number of forms contained in our Registration Pack. This includes a medical questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Applications to register will require to be supported by some form of identification which includes your name and date of Birth. eg, Driving Licence, Passport, Birth Certificate.

New patients aged over 40 years are offered a health check with a member of the Healthcare Team. This is to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
